Whenever players undertake this quest, they are given the Rathma book as a quest item, which explains why the letters on the book glow bright blue when it’s on the ground.
So, what triggers the book’s repeated appearance in Diablo 4? In most cases, it’s due to other players in the Fractured Peaks city. When these players accept the main quest with Neyrelle, they are granted the Death Harnessed: Theories of Rathma as a quest item. However, it can only be held within Kyovashad.

If any player holding the Rathma book attempts to leave Kyovashad, the quest item simply drops to the ground. This occurs regardless of whether you use a portal or exit through a main gate.
